Abstract

¿¬±¸¸ñÀû: ÀǵµÀû Àç½Ä¼úÀº Àç±Ù°üÄ¡·á¿Í Ä¡±Ù´Ü ¼ö¼úÀÌ °ï¶õÇϰųª ½ÇÆÐÇÑ °æ¿ì Ä¡¾Æ¸¦ À¯ÁöÇϱâ À§ÇÑ Ä¡·á ¹æ¹ýÀÌ´Ù. ¼º°øÀûÀÎ ÀǵµÀû Àç½Ä¼úÀ» À§Çؼ­ ÆÄÀý ¾ø´Â ¾ÈÀüÇÑ ¹ßÄ¡´Â ÇʼöÀûÀÎ °úÁ¤ÀÌ´Ù. À̸¦ À§ÇØ ÃÖ±Ù ÀǵµÀû Àç½Ä¼úÀ» À§ÇÑ ºñ¿Ü»ó¼º ¾ÈÀü ¹ßÄ¡¹ýÀÌ ¼Ò°³µÇ¾ú´Ù.

ȯÀÚ ¹× ¹æ¹ý: ºÐ´ç¼­¿ï´ëÇб³ º´¿ø Ä¡°ú º¸Á¸°ú¿¡¼­ ÀǵµÀû Àç½Ä¼úÀ» ½ÃÇàÇÑ 96¸íÀÇ È¯ÀÚ¸¦ ´ë»óÀ¸·Î ÇÏ¿´´Ù. Àç±Ù°ü Ä¡·á°¡ ½ÇÆÐÇÏ¿´°Å³ª ÇغÎÇÐÀû Á¢±ÙÀÇ ¾î·Á¿ò µîÀ¸·Î Ä¡±Ù´Ü ¼ö¼úÀÌ °ï¶õÇÑ °æ¿ì³ª ȯÀÚ°¡ °ÅºÎÇÑ °æ¿ì¸¦ ´ë»óÀ¸·Î ÇÏ¿´´Ù. ¼úÀü±³Á¤Àû Á¤Ãâ¼úÀ» ¾à 2-3ÁÖ°£ ½ÃÇàÇÏ¿© Ä¡¾ÆÀÇ µ¿¿äµµ¸¦ Áõ°¡½ÃÅ°°í µ¿½Ã¿¡ Ä¡ÁÖÀδëÀÇ ¾çÀ» Áõ°¡½ÃÄ×´Ù. °á°ú: 96°³ÀÇ Ä¡¾Æ´Â »ó,ÇÏ¾Ç ¼Ò±¸Ä¡ ¶Ç´Â ´ë±¸Ä¡¿´´Ù. ¿ÏÀüÇÑ ¹ßÄ¡ ¼º°øÀº 93% (n = 89)¿´À¸¸ç Á¦ÇÑÀûÀÎ ¼º°ø(Ä¡±ÙÀÇ ÀϺΰ¡ ºÎ·¯Áü)Àº 2% (n = 2), °ñÀýÁ¦¼úÀ» µ¿¹ÝÇÏ¿© ¹ßÄ¡ÇÑ °æ¿ì°¡ 5% (n = 5)¿´´Ù. ÀÓ»óÀûÀÎ ¼º°ø·üÀº 95%¿´À¸¸ç ÀüüÀûÀÎ ¹ßÄ¡ ¼º°ø·üÀº 100% ¿´´Ù.

°á·Ð: ºñ¿Ü»ó¼º ¾ÈÀü ¹ßÄ¡¹ýÀº ÀǵµÀû Àç½Ä¼úÀ» À§ÇØ ÀçÇö¼º ÀÖ°í ¿¹Ãø°¡´ÉÇÑ ¹ßÄ¡¹æ¹ýÀ¸·Î º¼ ¼ö ÀÖ´Ù.

Purpose: Intentional replantation (IR) is a suitable treatment option when nonsurgical retreatment and periradicular surgery are unfeasible. For successful IR, fracture-free safe extraction is crucial step. Recently, a new extraction method of atraumatic safe extraction (ASE) for IR has been introduced.

Patients and Methods: Ninety-six patients with the following conditions who underwent IR at the Department of Conservative Dentistry, Seoul National University Bundang Hospital, in 2010 were enrolled in this study: failed nonsurgical retreatment and periradicular surgery not recommended because of anatomical limitations or when rejected by the patient. Preoperative orthodontic extrusive force was applied for 2-3 weeks to increase mobility and periodontal ligament volume. A Physics Forceps was used for extraction and the success rate of ASE was assessed.

Results: Ninety-six premolars and molars were treated by IR. The complete success rate (no crown and root fracture) was 93% (n = 89); the limited success rates because of partial root tip fracture and partial osteotomy were 2% (n = 2) and 5% (n = 5), respectively. The clinical and overall success rates of ASE were 95% and 100%, respectively; no failure was observed.

Conclusions: ASE can be regarded as a reproducible, predictable method of extraction for IR.
